Description
- Coordinate and oversee client care plans to ensure their needs are met effectively.
- Provide excellent customer service to clients, families, and healthcare professionals.
- Maintain accurate records and documentation of client interactions and services provided.
- Schedule and manage appointments, ensuring timely service delivery.
- Act as the main point of contact for client inquiries and concerns.
- Collaborate with healthcare professionals to improve service delivery and outcomes.
- Monitor and evaluate client care plans, making adjustments as needed.
- Support team members with administrative duties as required.
Profile
A successful Care Coordinator should have:
- Experience in customer service within the healthcare industry.
- Strong organisational and time management skills.
- Excellent communication and interpersonal abilities.
- Proficiency in record-keeping and attention to detail.
- A proactive and empathetic approach to client care.
- Ability to work collaboratively with a team and independently.
