Kalwun Development Corporation Ltd is an Aboriginal & Torres Strait Islander community controlled organisation currently seeking to fill the following vacancy within our Seniors Servicesdivision.Kalwun Seniors Services is a provider of the Commonwealth Home Support Programme and Home Care Packages, and runs a day respite Wellness Centre.
About the Role
The Senior Care Advisor is responsible for leading a team of Care Advisors and guiding the co-ordination and delivery of individualised customer care plan development, ongoing management and outstanding customer service of Kalwun's Senior Services valued client base through competent care management in an office and field environment.
Position Duties
- Lead and guide a team of Care Advisors, ensuring culturally safe care management.
- Ensure compliance with funding and program guidelines, policies, and identify training opportunities.
- Assess client risks and implement safety measures to meet Aged Care Standards.
- Oversee caseloads, ensuring accurate quarterly and annual reviews.
- Facilitate team meetings, one-on-ones, case conferencing, and guidance on complex cases.
- Act as primary escalation point for feedback, following Kalwun's policy.
- Empower Care Advisors to make client-centered decisions in collaboration with stakeholders.
- Collaborate with the Cultural Practice Advisor to ensure cultural safety in care delivery.
- Oversee assessments, care planning, and reviews for CHSP, Home Care Packages, and NATSIFAC funding.
- Manage client budgets and funding, balancing care quality with cost efficiency.
- Deliver care in line with Kalwun policies, the Aged Care Code of Conduct, and relevant Standards.
- Relevant qualifications in Human/Behavioural Sciences, Allied Health, Nursing, Aged Care, or Community Services, or extensive care management experience.
- Strong understanding of Home Care Package, CHSP, and NATSIFAC frameworks, or willingness to learn.
- Experience working with complex client matters and addressing participation barriers.
- Ability to undertake administrative duties and governance.
- Proficient in Microsoft Office (Word, Excel, Outlook) and client/data management systems.
- Knowledge of the Aged Care and/orCommunity Sector.
- Familiarity with the Aged Care Standards, or willingness to learn.
- Understanding of Aged Care, Wellness, and Reablement principles.
