About the company:

Our client, a not-for-profit based just north of Melbourne CBD, is looking for a Senior Support Coordinator to join their team. This is a full-time role, working Monday – Friday. The company offers flexible work options – 3 days in the office and 2 from home.

You will be reporting into the Team Leader of Support Coordination and working alongside the Support Coordination team.

The successful person will facilitate complex and specialised support in line with an individual's NDIS plan, empowering participants to navigate the NDIS, address complex needs, and achieve sustainable outcomes through formal, informal, and community supports.

Key responsibilities:
  • Provide advanced support coordination and case management for individuals with diverse needs.
  • Develop and implement tailored strategies to help participants maximize their NDIS plans.
  • Mentor and guide team members to drive best practices and innovative solutions.
  • Build strong community connections to enhance service accessibility.
  • Ensure compliance with industry standards and contribute to service improvement.
You will have the following:
  • 3-5 years of experience in support coordination, case management, or a related field.
  • Strong understanding of the NDIS, disability legislation, and funding models.
  • Proven ability to develop support plans, navigate complex systems, and advocate for clients.
  • Exceptional communication and stakeholder engagement skills.
