The Community Care Tuberculosis Care Coordinator is part of a multidisciplinary team with a district-wide focus on TB treatment, prevention and refugee health. The role provides services related to prevention, control, detection, surveillance, education and treatment of tuberculosis and supports clinics in Queanbeyan and Goulburn, with outreach to other communities in the region. What You'll Be DoingAs the Tuberculosis Care Coordinator, you will deliver person-centered, specialist nursing care at an advanced practice level for TB patients in Southern NSW. This includes high-quality patient assessment, care planning and referral, and coordinating TB clinics in partnership with tertiary providers and General Practitioners to ensure a flexible, quality TB management and treatment service.
Additional Information- This position is open to Registered Nurses with a minimum of 3 years' experience or Clinical Nurse Specialists Grade 1 who would like to progress to Grade 2.
- The successful applicant is responsible for completing their BCG and TST qualification at their own cost.
- Scholarships may be available to support gaining immunisation accreditation.
- Current AHPRA registration as a Registered Nurse with post-registration qualifications in infection control, Public or Primary Community Health, or related field, with at least three years' clinical experience and NSW TB Service accreditation to administer TST and BCG vaccinations.
- Competence in multidisciplinary team-based speciality nursing care; ability to exercise extended autonomy in clinical decision making and leadership in nursing practice advancement.
- Effective clinical communication using information technology, written and verbal skills, and strong interpersonal abilities.
- Knowledge and application of evidence-based practice to advocate for and deliver person-centred care, with a commitment to quality improvement.
- Ability to supervise, delegate and negotiate to deliver quality nursing care, with leadership in coordination and delivery of care.
- Participation in professional development and maintaining competence as an authorised nurse immuniser (current TST/BCG accreditation) with ongoing TB-specific skills; willingness to mentor others.
- Current unrestricted drivers' licence and willingness to drive/travel as required, including long distances and overnight stays.
