Program Manager - Community Case Management

Under the supervision of the Associate Director of Community Health Services, the Program Manager has clinical, administrative, and supervisory responsibility for the implementation and operation of the Core Case Management and Scattered Site teams (collectively, Community Case Management). The Program Manager works closely with the Program Coordinators to ensure that goals and objectives specified for the programs are achieved within the program time limitations.

This is an exempt, non-union position. The hours of work are Monday - Friday, 8:00am to 4:30pm.

Required Qualifications
  • Master's degree in social work or a social service discipline with a minimum of five years of mental health, chemical dependency, and/or social services work experience, including at least one year of supervisory experience, or an equivalent combination of education and experience.
  • LSW/LPC required; LISW/LPCC strongly preferred.
