Our Opportunity

Are you passionate about helping vulnerable children in the community? Join Cafs, a values-driven organisation, as a Home Based Care Practitioner. This role is part of our Home Based Care Team. You will monitor and support the placements of children in Kinship and Foster care situations, working closely with carers and providing Case Management.

About You

We seek a motivated, values-driven individual with experience in case management and out-of-home care. The ideal candidate thrives when working both autonomously and collaboratively within a dynamic team.

Skills & Attributes

  • Tertiary qualification in Social Work, Welfare, Community Services, or similar
  • Experience working with vulnerable young people and case management
  • Understanding of Foster and Kinship care arrangements
  • Personable with the ability to build good working relationships and rapport

We provide over 50 programs across various service areas, caring for clients within communities across the Central Highland region.
