ABOUT THE ROLE
We are looking for exceptional Care Coordinators who are passionate health/aged care professionals looking to join our Caloundra, Caboolture & Gympie offices on a full-time basis. As Care Coordinator you will be an integral point of contact for workers and clients to provide support for the broader community team.

As a Care Coordinator you will be responsible for:

  • Building strong and meaningful connections with clients and their family.
  • Conducting comprehensive client assessments and development of care plans to support our clients to remain within their home.
  • Interacting and providing professional health and clinical care advice and direction to a team of home care workers.
  • Professionally liaising with clients and their families on a regular basis, addressing any concerns and service satisfaction.
  • Ensuring all practices are consistent within policy and legislative frameworks.
  • Maintaining up to date client records and information.

To be successful you must hold:

  • Formal tertiary qualification eg. Bachelor of Nursing or equivalent.
  • Current AHPRA registration as a Registered Nurse.
  • Demonstrated knowledge within the Aged Care sector.
  • National criminal history/police check.
  • Industry applicable licences (eg. AHPRA registration, BC/NDIS).
  • Valid First Aid and CPR certificate.
  • Minimum of 2 COVID vaccinations.
  • Open drivers licence and reliable transportation.
