Role Overview

Are you passionate about supporting others on their health journey? Central Coast Local Health District (CCLHD) is seeking a dedicated and compassionate Allied Health Assistant to join our Transitional Aged Care Program (TACP) multidisciplinary team at Wyoming Citigate Centre. This role offers a rewarding opportunity to collaborate with allied health professionals to enhance patient outcomes across hospital, community, and home-based settings.

About the Role

You will be part of the TACP, supporting older persons transitioning from hospital to home post-admission. The role may require moving between community-based work and our Transitional Care Unit (TCU) in Woy Woy to meet client care needs. Flexibility and adaptability within your scope of practice are essential.

About You
  • Hold a Certificate IV in Allied Health Assistance or relevant qualification, or be willing to undertake this training.
  • Possess a current, unencumbered NSW driver's license and be willing to travel across sites.
  • Have experience working in health settings with diverse patient cohorts.
  • Ability to prioritize, meet deadlines, and work within your scope of practice.
  • Strong communication skills and computer literacy.
