Social Worker - Grade 2 (General Medicine) – Permanent, full-time position based at St Vincent's Hospital Melbourne, Fitzroy. An opportunity for an experienced Grade 2 Social Worker to join our team on a permanent basis, supporting patients admitted to the General Medicine Units.
Responsibilities- Use skills to undertake multidimensional, comprehensive psychosocial assessments, apply clinical reasoning, and formulate appropriate complex intervention and discharge plans
- Liaise with patients, families and/or carers to coordinate, facilitate and communicate patient treatment and outcomes
- Actively participate in quality improvement and research activities
- Provide supervision and participate in clinical education programs as required
- Promote and develop positive relationships within the department, wider organisation and with community partners
- Bachelor Degree or Qualifying Master in Social Work
- Significant social work experience in an Australian hospital setting
- Current National criminal history check, or willingness to obtain
- Eligible for membership of the Australian Association of Social Workers

An opportunity currently exists for an experienced Grade 2 Social Worker to join our team on a permanent basis. At St Vincent's we have an approach to working with patients which puts them at the heart of their care. This position will work with patients admitted to the General Medicine Units at St Vincent's Hospital, Fitzroy. Applicants must have experience working in an inpatient hospital social work position.
