About us: Aged Care Assessment is a multidisciplinary team that assesses frail clients aged for Indigenous clients) to determine their care needs for residential, respite, community care, and rehabilitation programs, aiming to facilitate access to appropriate services through comprehensive, client-centered evaluations involving clients, carers, and providers across Central Queensland.
ResponsibilitiesAbout the role: The Aged Care Assessment (ACA) Senior provides leadership, training, and support to clinicians, conducting assessments in hospital or community settings per Commonwealth guidelines and the Aged Care Act 1997, with a focus on social work principles, comprehensive care planning, and ensuring quality, evidence-based practice through effective communication and collaboration within the public health system.
QualificationsAbout you: Hold qualifications and registration with the appropriate registration authority to practice in any of the following:
- Occupational Therapy
- Physiotherapy
- Social Work
- Registered Nurse
- Previous Aged Care Assessment experience would be an advantage.
Total remuneration value of up to $164,212 p.a. is comprised of:
- An hourly rate between $ $72.58 p.h.
