This position will enhance existing services to provide an assertive approach to referrals and consultation due to recent events related to isolation and restrictions in the community for the older population.
The role will undertake assessment, provide evidence based treatment and assertive care to Older Adults. The role will work in collaboration with mental health teams, aged care, General Practice and Primary Health Network services. The role will provide in reach to Residential Aged Care Facilities (RACF) for short term Mental Health treatment and care for older persons experiencing acute severe behavioural disturbance and psychological symptoms by preventing where possible the need for an inpatient admission.

Requirements

- Applicable vaccination requirements for a Category A role.
- Working With Children Check (WWCC)
- Current AHPRA Registration or Social Work degree and eligibility for the Australian Association of Social Workers (AASW)
- For nursing, a Registered Nurse with current AHPRA registration and relevant post-registration qualification and at least 3 years' experience working in the clinical area of their specified post-graduate qualification, or such other qualifications or equivalent experience deemed appropriate.
- Valid NSW Drivers Licence
