  • Salary Range: $67,538 - $92,227 per annum, plus superannuation

South West Healthcare (SWH) is seeking a passionate and committed Aboriginal Liaison Officer to join our team and improve health outcomes for Aboriginal and Torres Strait Islander people living with chronic and complex conditions.

This role offers an opportunity to support individuals, families, and communities in accessing culturally safe, coordinated care to achieve better health outcomes.

Please note: This position is not specifically identified as an Aboriginal position, but Aboriginal and Torres Strait Islander people are strongly encouraged to apply.

The Aboriginal Liaison Officer will help clients navigate the healthcare system and access necessary services, focusing on early intervention, self-management, and culturally safe care pathways for those with chronic illnesses.

This highly collaborative role involves working closely with hospitals, community health, aged care services, and Aboriginal Community Controlled Health Organisations (ACCHOs) to strengthen partnerships and improve outcomes.

Key responsibilities include:

  1. Advocating for Aboriginal patients and their families in acute and emergency care settings.
  2. Collaborating with clinicians to ensure culturally appropriate, respectful, and patient-centered care.
  3. Supporting clients in developing personalized, goal-oriented care plans.
  4. Fostering partnerships between SWH, ACCHOs, and other health service providers.
  5. Providing health education tailored to individual needs and promoting participation in self-management programs.
  6. Contributing to capacity building through education, training, and quality improvement initiatives.

Qualifications and experience:

  • Relevant qualifications in Nursing, Allied Health, or Aboriginal Health.
  • Strong skills in assessment, care planning, and service delivery.
  • Ability to work independently and collaboratively.
  • Excellent communication skills with a culturally sensitive approach.
  • Understanding of community supports, services, and networks.
  • Experience in acute or community health settings.
  • Effective time management and prioritization skills.
