About The Role The Chronic Health Nurse is responsible for navigating care of the patient and services that facilitate a person with a chronic disease to manage their health in a way that will result in optimal health outcome.
A creative, flexible and adaptable person who is able to think outside the box and be good problem solver whilst managing a vulnerable and complex cohort of patients.
An extended leadership role in the coordination of nursing and health care and passionate about quality improvement, whilst working within a multidisciplinary team.
Providing care in a range of modes and settings including home visits, MDT clinics, virtual care and group classes.
Collaboration with many external stakeholders including but not limited to Primary Health Network, NDIS, My Age Care, other Local Health Districts and Non-Government Organisations'.
About You Currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency.
Relevant post-registration qualifications and at least three years' experience working in the clinical area of their specified post-graduate qualification.
Current unrestricted NSW Driver's License.
Demonstrated effective time management skills and ability to prioritise competing workloads within a complex environment.
Demonstrated evidence of integration of theoretical knowledge and clinical skills, critical thinking and analysis in their daily practice as a registered nurse.
