About the role
The Department of Justice is seeking a dedicated and experienced Chronic Disease Nurse Coordinator to join the health services team within the Western Australian Corrective Services system.
This essential role focuses on improving the health and wellbeing of incarcerated individuals living with chronic conditions such as diabetes, cardiovascular disease, respiratory illnesses, and other long-term conditions.
Working within a multidisciplinary team, you will provide clinical leadership in the coordination of care, oversight, comprehensive assessments, contribute to developing and monitoring individual care plans, and support self-management through education and culturally appropriate health promotion.
As part of your role, you will collaborate closely with prison health services staff, general practitioners, mental health and allied health teams, and custodial staff to ensure a consistent and patient-centred approach to care.
You will also contribute to discharge planning and continuity of care as individuals transition back into the community, including liaising with community health providers and Aboriginal health services where relevant.
You will be responsible for liaising with the local health department to ensure patients are placed on appropriate registers to ensure continuity of care throughout their journey.
This position also forms part of the policy review team and will take ownership in informing and developing policies and procedures that relate to Chronic Disease Management.

Eligibility
Qualifications – the successful applicant must be eligible for registration in the category of Registered Nurse by the Nursing and Midwifery Board of Australia.
Permanent positions – To be eligible for appointment to permanent positions in the West Australian Public Sector you must be an Australian citizen or have permanent resident status in Australia.
WWC positions – In accordance with the National Child Safety Principles, people who work directly with children and young people, you must have a current Working with Children Check or be able to successfully apply for one to be eligible for appointment to this position.
Special conditions – it is also a requirement that the holder of this has a valid 'C' or 'C-A' class driver's license or equivalent.
While the position will be based in Welshpool, the successful applicant will be required to travel to various sites as needed.

Provide a comprehensive Resume / Curriculum Vitae, that clearly outlines your transferable skills, experience and highlights your key achievements including the contact details of two referees, one of whom must be your current line manager.
Provide a statement no more than three (3) pages, addressing numbers 2,3,5,6 and 8 of the Essential role specific criteria as listed in the JDF.
Please provide examples to support your claims.
Demonstrated significant senior nursing experience, knowledge, and clinical management of patients with chronic disease.

Demonstrated highly developed written and verbal communication skills including negotiation, facilitation, and interpersonal skills.

Demonstrated leadership and coordination of chronic disease program management, project management and organisation skills.
(5)
Skills and knowledge to develop strategies to prevent the onset of avoidable chronic conditions.
(6)
Evidence of high-level initiative and problem-solving skills including conceptual and analytical ability.
(8)
A copy of your current AHPRA Nursing registration as described in the above eligibility section.
A copy of your current and valid ' C' or ' C-A' class driver's licence or equivalent.
