About the role: Coordinate, lead, and participate in projects related to patient flow and care coordination across services.
Monitor patient wait times and discharge dates via electronic patient journey boards, identify key 'Waiting for What' (W4 W) and patient exit blockers.
Lead multidisciplinary meetings to ensure timely patient care, advise referrers, and escalate bed blocks.
Work within a dynamic team to provide quality care to community clients of the Palliative Care Service.
Provide evidence-based nursing care to diverse populations, including individuals, families, and communities.
Take a leadership role in nursing and healthcare coordination, offering expert assessments and triage to optimize outcomes, and provide feedback for improvement.
About you: Currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency.
Current NSW driver's license (Class C or above).
Relevant post-registration qualifications and at least three years' experience in palliative care.
Ability to work within an interdisciplinary team, with a commitment to professional development, effective communication, and interpersonal skills.
Strong integration of theoretical knowledge, clinical skills, critical thinking, and time management skills.
