Due to ongoing growth, we currently have an opportunity for an experienced Coordinator of Supports to join their team in Macksville.
The position of Support Coordinator is to provide individualised support to clients of the National Disability Insurance Scheme (NDIS).
The Support Coordinator will work with clients to implement supports within their plans to build capacity, manage the impact of their disability, and improve their health, well-being, and community participation.
Key responsibilities : Provide support to clients based on their NDIA plan, implementing all supports within the plan, including formal, mainstream, and funded supports.
Assist clients to understand their supports, access and use the client portal, find contacts, and select providers.
Co-design individualised plans and source support programs with clients, carers, and relevant stakeholders to meet the outcomes of their care plans.
Manage client issues and complaints with respect and fairness, following policies and procedures.
Prepare clients for their next plan review by evaluating current supports and exploring new options.
Support clients and their networks to understand NDIS processes, such as establishing provider agreements, managing budgets, and setting goals.
Maintain regular contact and record interactions.
Keep a working log of all contacts with clients, families, carers, and support services within Support Ability.
Prepare monitoring reports, documentation, and quality reports for internal committees as required.
Bill for supports provided by rostering in Support Ability to ensure payment.
Manage a caseload efficiently and coordinate tasks effectively.
Provide information on community resources and facilitate appropriate referrals.
What we're looking for : A minimum of five years' experience in disability services or a similar environment, with demonstrated expertise in complex disability support.
Relevant tertiary qualification in Disability, Individual Support, Community Services, or substantial equivalent experience.
Knowledge of human rights-based and person-centred approaches, including positive behaviour support.
Ability to manage a caseload of 20-30 clients.
Skills in coaching, mentoring, and team development.
Strong communication skills: written, verbal, and listening.
Capacity to implement change and foster innovation.
Financial management skills, including billable hours, P&L, and budgets.
Proficiency with systems and technology.
Excellent administrative skills: time management, task coordination, and efficient work practices.
Ability to work independently, monitor progress, and achieve outcomes.
Interpersonal skills to engage clients, families, and stakeholders effectively.
Current Working with Children Check – DWES and National Police Check.
