Your new role
The Adolescent Family Services suite of programs provides time-limited, tailored support to young people aged 12-17 years and their families or carers.
Our focus is on preventing statutory intervention and homelessness by addressing family conflict and relationship breakdowns.
We offer a range of interventions, from early intervention to intensive support, to help resolve issues and create lasting change.
As a Practitioner in the Adolescent Family Services program, you will play a key role in providing outreach-based assessments, mediation, case management, and tailored interventions to both young people and their families.
This is a highly rewarding and dynamic role where you will support young people in crisis, foster stronger family relationships, and assist in connecting them to crucial services in the community.
You will also:
Provide outreach-based assessments, mediation, and interventions to support young people and families, helping resolve issues and keep young people in the family home.
Develop Child and Family Action Plans, addressing complex issues like family conflict, parenting challenges, and high-risk behaviors.
Collaborate with schools, community services, and stakeholders, providing referrals for resources on family violence, mental health, and education.
Ensure compliance with child safety standards, risk assessments, and accurate documentation while participating in regular team meetings and professional development.
Work flexible hours as needed to meet the needs of families and young people, responding sensitively to diverse backgrounds, including Aboriginal, Torres Strait Islander, and LGBTQIA+ communities.

Your skills and experience
To be successful in your application you will possess a Bachelor of Social Work, Youth Work, or relevant tertiary qualifications and practical experience in case management.
You should have a deep understanding of the challenges faced by young people and marginalized families, particularly in the context of adolescence and family dynamics.
You will also have:
Ability to connect with young people and their families, build trust, and engage effectively.
Experience in conducting complex assessments, preparing detailed reports, and developing actionable plans in collaboration with clients.
Awareness and knowledge of working with diverse groups, including Aboriginal, Torres Strait Islander, CALD, and LGBTQIA+ communities, ensuring cultural safety.
Background in working with or for child protection services.
Well-developed interpersonal skills, confident in case meetings and presentations, and the ability to collaborate within a team.
Strong time management and organizational abilities to handle caseloads effectively.
