The Department


HITH provides acute nursing therapy to patients in the community as a substitution for care otherwise provided in the inpatient setting.

Alfred Health HITH cares for a diverse cohort of patients, within metropolitan Melbourne and across the state. HITH operates across the Alfred and Sandringham Hospitals, providing a seven-day-a-week service. Patients can be referred from The Alfred, Sandringham and Caulfield Hospitals.

The Role


Alfred Health Hospital in the Home (HITH) is seeking expressions of interest from experienced registered nurses for the position of clinical coordinator.

This senior nursing role coordinates care for patients in the community setting that would otherwise require bed-based hospital care.

The role involves workforce delegation and collaboration across a wide range of stakeholders including patients and carers, ward and programs leads, pharmacy, external service providers, and more.


Skills and Qualifications

_ Essential:
_


  • Registered as a Registered Nurse with the Nursing and Midwifery Board of Australia via AHPRA
  • IV cannulation accreditation
  • Computer and general IT literacy
  • A current Victorian Drivers Licence

_ Desirable:
_


  • Current experience within the acute and or ambulatory/community setting
  • Current experience in chemotherapy / oncology care
