About the role

  • Responsible to the TCP Service Manager for the provision of a task oriented welfare service focused on meeting the practical needs of patients and their families to find appropriate aged care placement and return to safe community living
  • Assist patients to receive benefits and services necessary to support hospitalisation, discharge and future care, by negotiating and brokering with the appropriate resource providers.
  • Responsible as the key person in the Placement Services for establishing and maintaining documented knowledge of aged care facilities, and for maintaining ongoing liaison with those facilities, patients and their families.
  • Provide assistance to all clinical and multidisciplinary staff in the placement of clients in residential aged care or community care facilities
  • Provides a welfare service to all patients assessed as eligible for entry into residential aged care.

Essential Requirements:

  • Current National Police Clearance (NPC)
  • Working with Children Check
  • Aged Care Screening check
  • Immunisation screening as indicated in the role description.
