The Home Health Platform is a leading provider of home-based Health and Aged Care services, committed to enriching the lives of individuals by delivering personalised support within the comfort of their homes.


The Opportunity


Australian Unity's Health Concierge and Care Partnering business unit is a critical new function that will lead the establishment and sustainable growth of the function to set the Home Health Platform on a path of integrating and streamlining disconnected activities to support the changing demands and needs of customer.

As a pivotal leader, the General Manager Care Partnering will lead a dedicated team to drive care coordination and service matching for Australian Unity's Home Health clients.


In addition, you will be responsible for:

  • Refining and optimising care coordination mechanisms, making them more agile, responsive, and tailored for Australian Unity's clients.
  • Creating a healthcare delivery system that stands as a beacon of precision and timely interventions, all the while delivering a stellar customer journey.
  • Proactively reviewing and driving growth opportunities within the care delivery ecosystem
  • Broadening Australian Unity's outreach and cementing its position in the healthcare market as a leading, bluechip care provider.
  • Driving a culture that embodies innovation and continuous improvement translating synergistic collaborations into measurable business successes.

About You


As a Care Executive, you will be excited by the opportunity to drive innovation around the advancement of Australian Unity's Home Health Platform.

Your care-centric mindset, combined with your ability to leverage data insights to mobilise care delivery, will be instrumental in ensuring every customer interaction is met with a care service that perfectly aligns with their needs, resulting in unparalleled health outcomes.

In addition to an extensive background demonstrating exceptional care delivery, you will also possess:

  • Evidenced experience in a Care leadership role managing high performing teams supporting the delivery of products and services to customers.
  • Experience working in the health industry with the ability to build strong relationships to drive scale and growth.
  • Familiarity with the Healthcare sector and/or Aged Care sector including its challenges, trends, and opportunities.
  • A strong working knowledge of Quality Standards, Community aged care and/or community health care accreditation frameworks.
  • Working knowledge of funding across Government and Private health funding programs.
  • Exceptional stakeholder engagement and management, with communication, analytical and problemsolving skills operational leadership skills
  • Experience fostering a cohesive, carecentric performance driven culture.
