Clinical Nurse Manager

Juniper Elimatta Residential Aged Care Home, situated just north of Perth near Mount Lawley, offers a warm and welcoming environment for 42 residents. Adjacent to Elimatta Retirement Living Village, it provides residents with access to courtyards and various lifestyle and therapeutic activities. Families can gather in the cosy lounge areas or the popular sunroom and join in activities. Supported by a compassionate and respectful team, residents receive individualised clinical care while continuing to pursue their own interests.

Your Role:

As a Clinical Nurse Manager, you will oversee clinical and care services, collaborating with the Residential Manager and a multidisciplinary team to ensure high-quality, person-centered care. In this role, you will provide clinical leadership, coordinate resident care, and facilitate the delivery of person-centered services at the site. Working with the Residential Manager and Allied Health practitioners, you will ensure all services are delivered in a manner that maintains residents' quality of life and respects their individual rights. The successful candidate will be offered a full-time position on a fixed-term contract until June 2025.

Key Responsibilities:

• Comprehensive clinical assessment and review of residents' care needs
• Provide leadership and promote a collaborative approach to clinical and care staff to ensure delivery of person-centered care
• Actively support and work cohesively with the clinical care team
• Support the Residential Manager in identifying training needs and performance management of clinical and care staff
• Facilitate and coordinate the admission process for new residents
• Review AN-ACC appraisals prior to being submitted to Medicare, analyzing and reporting any anomalies to the Residential Manager

Key Requirements:

• AHPRA Registration
• 5 years of experience as a Clinical Nurse Lead in an aged care setting
• Comprehensive knowledge of evidence-based clinical and care practices
• Demonstrated knowledge of continuous improvement, accreditation requirements, AN-ACC funding requirements, and documentation
• Experience with clinical problem solving and a commitment to delivering high-quality, resident-focused care
  Satisfactorily pass all relevant Key Personnel checks including Bankruptcy and Police Clearance
