An exciting opportunity exists at our Salisbury South for a Commonwealth Home Support Programme (CHSP) Coordinator.

Based in Salisbury South, as our CHSP Coordinator, you will monitor the clients' Helping Hand Agreed Support plan, which has been developed from the Regional Assessment Service (RAS) home support plan.

The CHSP Coordinator will ensure regular reviews are conducted with the client to ensure current services are suitable, the client is making progress towards their Agreed Support plan goals and changes in needs which is causing concern to the client.

You'll also make an impact by:

  • Monitoring and reviewing client progress regularly, as per Helping Hand's Home Care guidelines, adjusting Agreed Support plans as necessary to meet evolving needs.
  • Promoting the Organisation Plan to meet the ongoing and changing needs and preferences of existing and future clients.
  • Identifying opportunities for service growth and expansion and implementing new service direction.
  • Providing timely response to all switchboard/phone/email or other enquiries within organisational guidelines, and route clients accordingly.

What you'll bring

To succeed in this role, you will have:

  • Certificate IV Community Services or equivalent work experience.
  • A strong theoretical approach to working with ageing population and demonstrated practical application of this approach.
  • A current Driver's Licence and own car or access to vehicle
  • Valid NDIS Worker Screening clearance or be willing to obtain one.

You'll stand out from the crowd if you have some of the following:

  • Experience in the Aged Care industry
