SOCIAL SUPPORT ACTIVITY FACILITATOR
Part Time, permanent, up to 24 hours per week

  • Colac Area Health is a values based organisation providing a unique integrated health service comprising of over 500 employees, providing Acute Care, Aged Care, Community and Allied Health Services.
  • This role involves working as part of a team to organise and facilitate activities and programs on a day to day basis providing the highest level of care for participants attending Miller House 2 Miller Street Colac. You will provide supported activities, exercise and programs to promote independence, wellbeing and social interaction within a stimulating environment and foster a sense of motivation, selfesteem, belonging and companionship.

What's required?

  • Minimum Certificate 3 in Age/Disability or Allied Health Assistant; or equivalent; or currently studying to achieve same
  • First Aid Certificate Level 2 or willingness to undertake
  • Demonstrated rapport with older people and people with a disability, and an understanding of their needs
  • Experience of working with the frail aged as well as with participants with intellectual and/or physical disabilities.
  • Demonstrated understanding of the needs of frail aged or people with a disability, and the social experiences that will add value to their live
  • Excellent interpersonal and communication skills
  • Excellent planning and organisational skills, including time management
  • Knowledge of and experience with computer systems including MS Word, Publisher, Excel, Outlook and social media platforms
  • Capacity to work as a sole worker and cooperatively within a team
  • A positive, energised and engaging attitude
  • Demonstrated ability of a person centred approach
  • Experience with group work and managing group dynamics
  • Working with Children Check, Police Check, Drivers Licence
