This recruitment may be used to fill future positions via an eligibility list for permanent, full time, part time, temporary and casual positions.

This is a maternity relief position and should the permanent incumbent require an early return from maternity leave, this contract of employment will cease with two weeks notice. Maternity relief positions can be for a period of up to 12 months, with the possibility of continued employment for up to a further 12 months for maternity leave reduced hours, therefore there are no claims to permanency.


We have an exciting maternity leave position that provides the opportunity to make a difference to young people and their families and carers and learn from a skilled, supportive, multidisciplinary team.

The Social Worker - Youth Health will work within the Youth Health team to:

  • Provide intake and referral, holistic psychosocial assessments and shortterm counselling ensuring young people and their families are supported as partners in their own health care.
  • Actively support and guide young people with multiple vulnerabilities and/or chronic and complex healthcare needs to navigate and engage with appropriate support services.
  • Provide outreach activities to engage young people most at need, including in schools and in health facilities.
  • Maintain a strong working knowledge of key support services for young people on the Central Coast.

You will be well supported within a multidisciplinary team navigating and supporting vulnerable young people to engage with specialist and mainstream health care providers.

You are passionate about client centred care and have a solid understanding of key support services for young people, particularly those with multiple/complex needs, in the local area.
