We are currently seeking a team member for the Permanency Case Management Team to provide Case Management to young people in Out of Home Care (OOHC).


In this role, you will work as an integral member of the Permanency Program and report directly to the Permanency Team Leader.

The Permanency Case Manager is required to work within the organisation's strategic objectives, values, and outcomes.

You would be responsible for promoting Marymead CatholicCare's services and delivering high quality services to children, young people, and families.

This is a full-time temporary (12 Months) position based in Goulburn, working as part of a team which involves direct contact with carers, children, young people, and families.

Ideal Person

The ideal person we are looking for has:
Flexibility to travel and work outside of core business hours and on weekends (as required).
Knowledge of the Out of Home Care systems.
Experience of providing case work to families, children, and young people.
Knowledge of developmental needs of children and young people.
High level of written and verbal skills.
High level of skills in report writing.
Ability to understand, respond to, and resolve child/young people crisis.
Provide support to the Carer Development teams to facilitate transitions to placements.
Provide additional support to families and young people during times of personal crisis.
Complete necessary data collection, case notes and reports within required timeframes.
Participate in and organise events.
Carry out general office and driving duties.
Maintain statistical records within the organisation.

Maintain professional knowledge and continue to develop knowledge and expertise in relation to professional skills and professional legislative and regulatory requirements.

Liaise with internal and external professionals to complete assessments or make referrals.

Assist in the maintenance of client records to meet privacy, confidentiality and security standards, client file audits and transfer of files.

Follow and comply with policy and procedures.
Participate in professional supervision including peer and supervisor review of professional practice where required.
Engage in probation and annual reviews.
Undertake activities to develop knowledge and expertise in relation to professional skills.
Complete HR forms/reports, including timesheets.
Participate in staff meetings and staff training; and contribute to the delivery of staff orientation.
Comply with and monitor adherence with the Work Health and Safety policy.
Under direction, participate in quality and statutory audits; and
Participate in activities supporting continuous quality improvement.

To be considered for this role you will be required to hold:
A current driver's License.
A Working with Vulnerable People Card (ACT); or
A Working with Children Check (NSW)

Eligibility to be added to the NSW Office of Children's Guardian (OCG) Residential Care Workers Register (RCWR) at time of reference check.

You will also be required to undergo a National Police Check.
