Overview:

  • The Aboriginal Health Practitioner Rheumatic Heart Disease (RHD) is responsible for coordinating, delivering, and monitoring programs and services that support Aboriginal primary health care services, patient management and the coordination of clinical care for Aboriginal communities. In particular the improved delivery of secondary prophylaxis (preventive antibiotics) for patients with acute rheumatic fever (ARF) and RHD, to support the Department's commitment to reducing morbidity and mortality associated with RHD in South Australia.
  • The role contributes to the Council of Australian Governments National Partnership Agreement on Closing the Gap in Indigenous Health Outcomes health related targets.
