Social Worker / Case Manager

Hunter Brain Injury Service (HBIS)

About the role:

  • Provide brain injury rehabilitation to clients in a range of service models, including clinic based, outreach, community residential (Transitional Living Unit) and telehealth in order to meet the individual needs of clients and to maximise service efficiency.
  • Work collaboratively with various service providers and agencies to deliver a personcentred communityfocused rehabilitation program with a strong focus on reintegration to the home, community and work environments.
  • Negotiate and act as an advocate for the client to deliver the most efficient rehabilitation program that remains client focused.
  • Use the NSW Brain Injury Rehabilitation Program (BIRP) case management model to deliver a rehabilitation program that maximises client outcomes.
  • Learn how to work with Lifetime Care and other insurance funded participants.

Where you'll be working:

  • The Hunter Brain Injury Service (HBIS) provides specialist rehabilitation to people 1865 years of age who live in the Newcastle, Lake Macquarie, Hunter Valley, Central Coast, Port Stephens and Taree.
  • Work with individuals who have experienced a traumatic brain injury or an acquired brain injury.
  • Rehabilitation care may occur in the transitional living unit for up to 5 people (Monday Friday), an outpatient clinic and / or an outreach service.
  • The large multidisciplinary team are specialists in the area of brain injury, and act as a resource for the wide HNE regarding care and support.
  • The service in based at Bar Beach Newcastle, an amazing work location only 250m to the beach.
  • There is parking onsite and the Junction is only 5 minutes away

  • You take a holistic, innovative and empowering approach to care acknowledging the wider social, psychological, societal and cultural factors that may affect the client during their rehabilitation and transition back to home, the community and work environments.
  • You are a passionate interdisciplinary team member who actively contributes to a positive team culture and works from a clear ethical and valuesbased framework
  • You are savvy in developing working relationships within and outside the team
  • You are enthusiastic about quality improvement and your professional development
  • You can act as a resource for other social work staff and wider team as well as supervising students

How you will be supported:


The service is one of 15 dedicated specialised rehabilitation services across NSW where you will have access to a range of supports, supervision, education and training.

In addition to this, you will be supported by your manager and your peers both inside and outside of the service including the broader Social Work team in Greater Newcastle Sector of Hunter New England Health.

All NSW Health workers are required to have completed a primary course of a COVID-19 vaccine which has been approved or recognised by the Therapeutics Goods Administration (TGA).
