The Advanced care finder program provide specialist support and intensive assistance to help people to understand and access aged care and connect with other relevant supports in the community.

It will specifically target people who have one or more reasons for requiring intensive support to interact with My Aged Care, access aged care services and/or access other relevant supports in the community.


Working as the Team Leader, you will support a small team to deliver the implementation and ongoing services delivery of CatholicCare Victoria's Advanced care finder services.

You will provide an integral role in providing specialist and intensive assistance to help people to understand and access aged care and connect with other relevant supports in the community.


Specifically, the position is responsible for:

  • Managing a varied client caseload through both supervision of staff and personal direct service delivery including managing a waiting list/active hold where required.
  • Participating in the formation of programs and work practices.
  • Supporting the Manager, Homelessness and Housing Support in ensuring the effective implementation of workforce planning and recruitment.
  • Liaising with other services about demand management, referrals and service delivery issues.
  • Undertaking responsibility for local site management issues including health and safety, administration and related matters as required

To be considered for this position, applicants require:

  • Relevant tertiary qualification, preferably in Social Work, Psychology, Human Services or Community Development or similar related discipline
  • Demonstrated experience in the delivery of health, social or community services
  • Demonstrated experience in providing professional supervision to clientfacing staff in the delivery of human services programs
  • Strong understanding of a variety of theoretical frameworks and intervention strategies, including working with children, young people and families
  • Sound understanding of relevant legislative requirements and funding body frameworks and guidelines
  • Demonstrated capacity to build and maintain effective relationships with a broad range of stakeholders
  • Proven skills in managing staff engaged in service delivery, case work in a multidisciplinary setting
