Role description


HARP is a multidisciplinary service that supports patients with chronic diseases to maintain optimal achievable wellness and to reduce emergency presentations and admissions to hospital, mostly through boosting patient self-efficacy.

The team consists of nursing staff, pharmacists, physiotherapists, occupational therapists, dieticians, social workers, physicians, geriatricians, emergency physicians and palliative care clinicians.


The Talk To You Tomorrow (TTYT) service is a post-discharge clinical follow-up model that is collaboratively operated by HARP and General Medicine, which specifically targets patients who have been discharged from hospital General Medicine units in the last 1-3 days.

The service proactively makes contact with patients and carers during the initial transition from hospital to home, largely to ensure that the advice provided by the hospital team has been comprehended and retained by the patient and carer, and to institute support services if such need becomes evident.


The purpose of the HARP Consultant position is:
to provide excellent clinical care.

to provide leadership for all members of the team.

to take part in the design and implementation of innovative healthcare models and technologies.

to ensure operational requirements, such as activity recording that is linked to funding, are met.

to support the organisation's teaching, research and clinical governance programs.


The HARP Consultant position is suited to Senior Medical Staff (SMS) from a variety of different specialties, including but not limited to General Physicians, Geriatricians, Emergency Physicians and Palliative Care Physicians.

In addition, doctors with fellowships in their relevant Specialist College, including general practitioners, would be suitably qualified.

Current registration with AHPRA

Highly developed interpersonal and leadership experience

Essential:
Hold Fellowship of a relevant specialist college

Essential:
Capacity to deliver care in the hospital (all EH campuses) and in the ambulatory setting

Essential:
Has significant clinical experience in the area of chronic and complex care
