ROLE AND RESPONSIBILITIES

  • Providing support in communitybased settings, group homes, respite, participant's homes
  • Providing support based on a personcentered approach, supporting inclusion and participation
  • Providing support to assist clients with daily life tasks as well as community participation
  • Working effectively both in a team and with different client ratios such as 1:1, 1:3 etc
  • Building and maintaining positive relationships with stakeholders
  • Being available to work a variety of shift types, including early morning, afternoon, evening, weekends, sleepovers and active nights
  • Creating and complying to an Individual Support Plan
  • Complying with all recording requirements, including but not limited to: recording medication administration, completing incident reports, completing participant progress notes, updating participant support plans and accounting for cash expenditure
  • Attending and participating in scheduled supervision meetings with Service Manager or Operations Manager
  • Ensuring own hours worked are accurately recorded and reflect actual hours worked
  • Taking reasonable care for your own health and safety and for that of others in the workplace by working in accordance with legislative requirements
  • Adhering to all Sunny Pine Home Care policies and procedures and NDIS guidelines and regulations
  • All other duties as directed by Management
  • Work a range of different shifts including day, evening and overnight.
  • Travel to assist participants with services in group homes, personal homes, facilities and more

To be eligible to accept this role you will have provided the following:
 NDIS Workers Screening Check (includes National Police Check)
 International Police Check (where applicable)
 Evidence of current Working with Children's check (where applicable)
 Evidence of a current Driver's License (International or Australian whichever is applicable)
 Signed and witnessed Statutory Declaration Form
 Evidence of relevant Qualifications
 Evidence of your right to work in Australia (Visa, unless you are an Australian Citizen or Permanent
Resident)
 Current First Aid Certificate - Level 2
 NDIS Worker Orientation Module Certificate
 A smart phone device
