About the role:


You will be responsible for ensuring the delivery of flexible and responsive services tailored to an individual's needs and interests, whilst also maintaining their independence and freedom of choice.


Performance Objective -To assign quality professional care for clients that enable them to remain independent in their own homes.

  • Ensure the care and wellbeing of our clients.
  • Clients/Families are happy.

Client Responsibilities-

  • Provide inhome assessments, referrals, care plan reviews & goal planning for clients.
  • Client budget management & reporting.
  • Excellent time management with client caseload.
  • Respond & communicate effectively with clients re: changing care needs.
  • Conduct regular visits with clients in person and/or over the phone to ensure the highest level of care is being attained while also determining any changing needs or requirement for additional services.
  • Answer queries, requests, complaints, and feedback relating to the delivery of care and services and/or the reconciliation of financial statements.
  • Advocate for client's needs.

Who are You?:

  • Have relevant qualifications in Social Science, Nursing, Allied Health or related field is desirable.
  • Case Management experience, using a person centred, strengthsbased practice framework.
  • Can work in a team and build positive relationships.
  • Demonstrate compassion and empathy
  • Demonstrated experience working within Government Guidelines & Regulatory Standards.
  • High degree of computer competency with an ability to learn new software programs.
  • Time management skills and attention to detail.
  • Have an understanding and experience assisting clients within the aged care sector.
  • Current NSW driver's License.
