The Hunter Valley Transitional Aged Care Program (TACP) is a multidisciplinary team dedicated to supporting aged care clients in enhancing their health and well-being after hospital discharge. Comprising Allied Health and Nursing professionals, the TACP team works closely with clients and their families, utilising an enablement-focused approach to provide slow-stream therapy. This method aims to improve clients' function, independence, and safety at home.

Operating as a home visiting service tailored to patient needs, TACP is available from 8:00 am to 4:30 pm, Monday through Friday, year-round. This role offers a temporary position of 32 hours per fortnight, with four shifts scheduled across the two-week period. Where you'll be working

Located in the stunning lower Hunter region, renowned for its picturesque wilderness areas and distinctive wineries, our health service offers a unique and enriching environment. The Physiotherapy position is an integral part of the Transitional Aged Care Program (TACP) at Hunter Valley Community Health, based at Kurri Kurri District Hospital.

As a member of the TACP team, you will contribute to improving the health and well-being of aged care clients post-hospital discharge. This role allows you to work in a supportive community health setting while enjoying the natural beauty and cultural richness of the Hunter Valley. What we can offer you:

In this role, you will collaborate with a dynamic team of Nursing staff, Physiotherapists, Occupational Therapists, and Social Workers to assess and support clients in achieving their goals. Enjoy comprehensive face-to-face training and ongoing support from the Service Manager, Department Senior, and TACP Physiotherapists to ensure a smooth transition into your role.

Requirements:

  • Physiotherapist with current Authority to Practice with AHPRA

Additional Information:

  • An eligibility list will be created for future temporary part-time vacancies.
  • To be eligible for employment in this position you must be an Australian Citizen, or permanent resident of Australia, or a New Zealand citizen with a current New Zealand Passport, or hold an appropriate visa that allows you to live and work in Australia. Employment of a temporary visa holder will only be offered in line with the conditions and expiry date of your visa.
