About the Position


The Mental Health Support Worker is responsible for providing high-quality, responsive, and person-centred support working with external care teams and families.


Working with individual's program plans, the position will support participants' lifestyle choices and assist to develop skills and strategies to better manage their mental health, access community resources, enhance their everyday living skills, and to achieve their mental health and well-being goals.


The key responsibilities are to:

  • Provide support with daily living skills and community activities to meet individual needs of our participants which may include but not limited to housekeeping, personal hygiene, administering medication and transportation to appointments and community activities.
  • Keep up to date with participants needs and care plans, including changes to participants care needs.
  • Experience of collaborating with multidisciplinary clinical teams to deliver comprehensive care plans.
  • Facilitate community inclusion and participation, dependent on individual needs, abilities and preferences, and support participants to establish and maintain relationships and interests through access to community activities and services.
  • Provide regular feedback to the Practice Lead/Regional Manager/On Call Manager of issues that require follow up relating to the residents, their families and external supports, other UFC employees and community access activities.

About You
You will thrive and succeed in our team by bringing the following experience to this position:

  • Qualification in disability, mental health, or related field (desirable)
  • Preferable 2 years' experience in mental health and complex/challenging behaviours.
  • Demonstrated confidence in maintaining professional boundaries and restrictive practices.
  • Ability to make sound decisions under pressure and deescalate crises.
  • Effective written and verbal communication skills and a demonstrated ability to write clear case notes.
  • Have sound emotional intelligence and communication skills including patience, empathy, tact and reflective listening.
  • NDIS Worker Screening Clearance (obtained from Service NSW)
  • Current First Aid Certificate
  • Unrestricted Australian Driver's License
  • Covid Vaccination Certificate(strongly encouraged)
