Our mission here at SWAMS is to provide high quality, culturally appropriate and holistic healthcare which builds strong healthy Aboriginal communities. The Community Connector will contribute to this mission by helping Elders and their families understand their aged care service entitlements, registration, costs, and liaise with age care assessors to ensure Elders are well-supported throughout the assessment process.

As a member of the Access team, you will:

No qualification is required for this role, however specific requirements of this position include but are not limited to:

Essential Criteria

  • Must reside in the South West and have local knowledge about the community.
  • Ability to take direction, determine priorities and manage workloads in order to meet agreed timelines and objectives.
  • Good communication skills, both written and spoken
  • Demonstrated ability to work as a team member and independently with minimal supervision.
  • Current WA C or CA Class driver's licence, with the ability to undertake outreach to the Greater Bunbury, South West region, and lower south Wheatbelt.

Desirable Criteria

  • Aboriginal or Torres Strait Islander people are encouraged to apply
  • NDIS Worker Screening Check
  • National Police Check

You can expect to be remunerated with an attractive package which includes, above award wages, 11% superannuation, a generous salary packaging opportunity of up to $15,900 and an annual corporate wardrobe allowance.
