Role Details
The Specialist Support Coordinator is a role within DHS Disability Services and is accountable to the Senior Specialist Support Coordinator for:

  • working collaboratively with clients, families and carers to improve supports through the NDIS (or other funding models) in a way that is respectful of people' human rights, puts the client at the centre of planning, and aims to improve quality of life for clients living with disability
- addressing complex barriers impacting a client's ability to implement their plan and access appropriate supports
- assisting clients to reduce complexity in their support environment and overcome barriers to connecting with broader systems of supports as well as funded supports
- negotiating appropriate support solutions with multiple stakeholders and seek to achieve well-coordinated plan implementation
- assisting stakeholders with resolving points of crisis for clients, assist to ensure a consistent delivery of service and access to relevant supports during crisis situations
- supporting participants of the National Disability Insurance Scheme (NDIS), in accordance with goals outlined in their NDIS Plan, providing a service that aligns with the NDIS Practice Standard and Quality Indicators provided by the NDIS Quality and Safeguards Commission, including restrictive practice standards.

Key Outcomes and Accountabilities

  • Provide support and information to clients, families and carers to understand the plan, connect with and establish supports and services, design support approaches, build capacity to be able to maintain engagement with supports.
  • Build capacity and resilience in clients including development of plan to prevent and mitigate and manage times of crisis.
  • Guide and support clients, families and carers to implement coordinate, monitor and review their approved NDIS Plan.
  • Research and coordinate a range of supports to assist client to achieve their individual goals through engagement of formal, informal, community and mainstream services.
  • Engage service providers with clients and support engagement is maintained, and provide information and support to clients, their families and carers enabling them to monitor the use of their supports within their plans.
  • Develop and work within Service Agreements with each client to deliver supports on time and within budget.
  • Assisting in planning, implementation and reporting on services; contribute to research, service development and quality improvement activities within the team.

Any other responsibilities in line with the classification level of the role as assigned by Line Manager and/or the Department.

Essential Qualifications
***Incumbent must hold an appropriate degree or equivalent qualification in Disability & Developmental Education, Occupational Therapy, Psychology, Social Work, or Speech Pathology which entitles registration or full membership with the relevant allied health profession's board. Occupational Therapists and Psychologists must have full registration with the relevant profession's board and maintain registration.
