Our HomeCare Facilitators in our Brisbane South team are supportive and collaborative with our clients whereby, all care and decisions are centered on both needs and wants.
In addition to playing an integral role in the planning and implementation of care plans, you will also provide support to care staff and manage incoming referrals.
Our Home Care Facilitators manage:
- Provide service coordination, assessment, care planning and review, for lowlevel support programs to ensure effective coordination and delivery of care services to customers
- Coordinate the planning and implementation of a Model of Care that provides customercentric quality care services in an environment of innovation.
- Support the Community Care Manager in recruitment, selection and orientation activities for employees and volunteers
- Manage a prescribed caseload including assessment and care planning to remain within budgetary constraints of the service users designated program(s)
- Supervise and support staff reporting to this position, including competency assessment and identification of training needs
- Minimum Certificate IV in Aged Care / Individual Support or other
- Experience in community aged care
- Experience in support roles directly overseeing staff
- Confident in the use of computers
- Capability to manage referrals
- Current Australian driver's license and own motor vehicle
- Current Police Check (willingness to obtain)
- Working rights in Australia
- 2023 Flu Vaccination or intent to get one
