Our Support Coordinators are dedicated to leading and energising teams to provide great service. Client relationship management is at the heart of everything we do.

Together with our Client Service Managers, our Support Coordinators partner with clients to coordinate their supports and help them make the best use of their NDIS Plan.

The role
At the heart of this role is second to none client relationship management.

Together with our Client Service Managers, our Support Coordinators will work with their clients to have choice and control over their supports and goals.

We're growing and specifically need Support Coordinators with experience in supporting people with complex needs, mental health and behaviours of concern in the specified location(s).


The role involves dealing with internal and external stakeholders and making a real difference in the lives of people with disability, supporting them to have the greatest sense of choice, control and confidence under the NDIS.


Here's what you can expect:

  • Ability to interpret NDIS Plans, organise the required supports, break down information for Clients so it's easier to understand each bit of the plan and funding.
  • Ability to communicate with Clients in their preferred communication style.
  • Develop and maintain positive, respectful and supportive relationships with our Clients and the important people in their lives.
  • Work sidebyside with Clients, both new and current to foster and promote positive outcomes.
  • Build client's capacity to maintain relationships, manage service delivery tasks, help clients live more independently and be included in their community.
  • Provide ongoing support, mentoring and clientspecific guidance for Value Care's care staff.
  • Maintain uptodate client records and monitor funded support budgets, as required (don't worry, you'll be working with our friendly finance team for this)
  • Contribute to the continuous improvement of Value Care's policies and procedures
  • Build a strong network of providers and ancillary supports that may be needed to pursue the goals of the client's NDIS plan.
  • Participate in our inhouse oncall after hours as required.
  • Stay ethical and follow the fine line and conflicts this role may come across

To do a great job, you'll need:

  • A successful track record in disability, mental health or community service
  • this must include previous experience working as a Support Coordinator; or in a similar capacity; or possess a sound knowledge of and networks amongst clients, support providers and community resources.
  • Experience working in organising direct supports for people with complex needs who display behaviours of concern.
  • Exposure to SIL and SDA clients, as well as at home supports.
  • Ability to think creatively and remain flexible and responsive to change.
  • Understand and/or have experience working directly with clients with behaviour issues
  • A passion for clientfocussed care and a collaborative approach
  • Active listening skills and the ability to build meaningful relationships with clients and care teams.
  • Be comfortable using (or learning to use) our cloudbased apps for organising supports or record client interactions.
  • Ability to interpret NDIS Plans as well as strong experience in building or nurturing networks to avail supports required in the client's Plan.
  • Relevant tertiary qualifications and/or experience in disability/community services/case management (or equivalent).
  • Full Australian driver's licence and preferably a suitable vehicle (we'll reimburse you for your use or else provide our own vehicle for client related engagements). You'll be assigned to teams in a specific territory, which means some travel. You'll be comfortable covering some distance for your role and also coming into our Local office on a periodic basis.
