The BHN Community Care team is seeking an experienced Exercise Physiologist to provide individual and group programs primarily to clients who reside in Supported Residential Services and other insecure housing, who may experience complex psychosocial needs such as homelessness and significant mental illness. The position will promote exercise to prevent and manage chronic disease, and to improve social inclusion. Individual and group work will be offered in both outreach and onsite settings.

Responsibilities:

  • Develop group and individual exercise programs that respond to the needs of SRS and insecure housing clients, and general client groups in both outreach and onsite settings.
  • Assertively engage with, and link SRS and insecure housing clients into group programs.
  • Triage SRS and insecure housing clients for physiotherapy need, and where appropriate, refer and collaborate with BHN program staff regarding client care.
  • Build new initiatives through partnering across the organization, and with external agencies.
  • Encourage group participants towards a self-sustaining and self-management model.

Requirements and attributes:

  • Registration with Exercise and Sports Science Australia as an Accredited Exercise Physiologist.
  • Minimum of two years post-graduate Exercise Physiology experience.
  • Experience working with clients with complex psychosocial needs and significant mental illness.
  • Demonstrated ability to provide exercise programs to aged and young people with a range of acute or chronic conditions, multiple and complex needs.
  • Highly developed interpersonal, communication and negotiation skills.
  • Ability to work both independently and as a member of a multi- disciplinary team.
  • Demonstrated skills in networking, liaison and the development of strong links with relevant service providers and users.
  • Commitment to, and enthusiasm for working with the target group.
