Community Connect Worker (CCW)
The Community Connect Worker (CCW) will work with individuals and families to assist them in the identification of barriers that prevent them from achieving lives that are safe, healthy and fulfilling.
The CCW will achieve this by assessing client needs and referring them to specialist services such as those that support people with complex needs.
KEY RESPONSIBILITIES:
- To provide timely and accurate information, advice and referral for the community with a focus on vulnerable individuals, families and populations;
- To facilitate access to more targeted services for vulnerable people and families;
- To provide integrated and coordinated responses as part of a case coordination model;
- To facilitate support to individuals, families and groups, including information, advice and referral;
- To respond quickly and effectively to arrange emergency assistance when required;
To develop and maintain close working relationships with specialist services across the region for referrals, support and assistance;: - To establish appropriate processes to managing the referral out of service users whose needs would be more appropriately addressed elsewhere. e.g. ATODS, mental health etc;
- To liaise and network with government departments, other agencies and LOMCC staff;
- To contribute to the identification, development and implementation of quality and service improvement activities including effective risk management in order to enhance the delivery of safe and efficient services;
- To prepare and present monthly reports outlining details of the service outcomes.
- Data collection, collation, entry and maintenance of information on relevant resources and data bases, including computerised management systems;
- To understand and conduct all duties in compliance with Department of Communities, Housing and Digital Economy Service Agreement and LOMCC policies and procedures,
- To comply with LOMCC WHS policies and take reasonable care of own health and safety and that of other personnel who may be affected by your actions;
ESSENTIAL SKILLS AND ABILITIES REQUIRED:
- Degree or diploma qualification and/or relevant experience in community services (minimum of 3 years)
- Positive notice to Work with Children (Blue Card)
- Current
- Criminal History Check/Exemption (Yellow Card) or the ability to attain one
- Current
- Commitment to social justice and community development
- Ability to work in team environment and autonomously
- Knowledge of community resources; or ability to acquire such knowledge Cultural awareness and sensitivity;

SELECTION CRITERIA - ALL APPLICATIONS MUST ANSWER ALL OF THE FOLLOWING QUESTIONS AND INCLUDE THEM IN THE EMAIL.
Q- Demonstrated interpersonal and communication skills
Q- Demonstrated understanding of cultural awareness and of the issues that impact on disadvantaged members of the community
Q- Tertiary qualifications in Community Development, Human Services, Social Work and/or behavioural/social sciences with a minimum of three years relevant industry experience or substantial experience
Q- Demonstrated competency in liaising and networking with individuals, families and community groups for the purpose of supporting of referrals and connections
Q- Knowledge of support programs and allied health facilities in the area for the purpose of mediation and referral.
Q- Demonstrated ability to work autonomously, maintain motivation and enthusiasm to develop and maintain appropriate networks within the community and a wide range of community and government stakeholders.
Q- Demonstrated organisational and time management skillsQ- Demonstrated high level of communication skills including oral and written, IT, report writing, submission writing and acquittal skills
ACCOUNTABILITY:
To the Centre Coordinator of Leichhardt - One Mile Community Centre
HOURS OF WORK: 38 hours per week (Monday to Friday)
SALARY:
Level 5.1 SACS Award ($44.92/hr) Plus applicable Allowances
