Your opportunity
You will provide effective and high quality care coordination and management services, to support older people living at home.

Specifically, you will manage allocated cases within Home Support Services including the management and supervision of Home Care Support Workers, and be the key liaison point for other services providing care and assistance to clients.

You will be the advocate for client's rights with regards to access and quality care.


You will work with clients and their families to design appropriate Care Plans to meet their needs, provide them with prompt contact and ongoing reviews to ensure that their care needs and goals are met and that services are adjusted as needed.

Requirements for success


The care Manager will possess a good understanding of Home Care Packages and NDIS along with Tertiary qualifications in Social Work, Social Science, Allied Health or Nursing.


  • Demonstrated experience in case management in the aged care sector.
  • Knowledge of relevant legislation and its implications for clients.
  • Understanding of personcentred approach to client care.
  • Ideally you will have the ability to monitor, supervise and direct staff in the management of client needs.
  • A full driver's license
