Job Opportunity: Care Manager

Overview

We are thrilled to invite highly qualified candidates toapply for the pivotal role of Care Manager. This position serves as thecornerstone of our client relations, working in symbiosis with our participantsand support workers to provide superlative, goal-oriented care in alignmentwith NDIS guidelines.

We are seeking somebody for a Permanent Part-Time positioninitially, with an FTE of We will also consider a dual contract fora support worker role for those seeking additional hours.

Scope of Responsibilities

As a Care Manager at Flourish Support Services, you will beentrusted with a multifaceted range of duties that are crucial to our dailyoperations:

  • ParticipantRelationship Management: Maintain impeccable communication andcultivate positive relationships with all participants.
  • ClinicalOversight: Collaborate on the development and execution of customizedcare plans, supervising the provision of services by support workers.
  • ClientCommunication: Promptly respond to client inquiries and concerns.
  • StaffDevelopment: Spearhead training modules and ongoing developmentalprograms for support workers.
  • ClientOnboarding: Implement comprehensive procedures for welcoming newparticipants to our services.
  • Documentation:Assiduously manage client files to ensure all records are up-to-date andcomply with regulations.
  • StakeholderCollaboration: Work closely with external stakeholders such ashealthcare providers, insurers, and regulatory bodies.
  • OperationalFlexibility: Assume rotating on-call responsibilities as part of ourcontinuity of care commitment.
Requirements
  • Valid Paid Blue Card or willingness to obtain
  • NDIS Worker Card or willingness to obtain
  • NDIS Worker Orientation Module or willingness to obtain
  • COVID-19 Infection Control Certificate or willingness to obtain
  • Ability to participate in rotating on-call responsibilities
  • Proficient in computer operations
  • Exceptional problem-solving capabilities
  • A valid driver's license and registered vehicle
Desired Qualifications
  • Diploma in Community Services or equivalent
  • Minimum 2 years of industry-relevant experience
  • Proficient understanding of NDIS regulations and practices
