About the Role
Community Solutions is a registered NDIS provider and specialises in SLES. The Support Worker will provide support for participants with an NDIS plan for SLES. This will be conducted in a supportive, non-judgemental approach to facilitate and support the participants journey towards employment. This will include support for participants to achieve greater independence, socialisation, education, and other identified goals.

Success Profile:

  • Provision of one on one NDIS supports for participants that have NDIS plans
  • Participate in the development and implementation of personalised plans
  • Maintain a sound knowledge labour market needs
  • Provide support and guidance to individuals to make their own decisions and improve capacity building skills
  • Provide direct and personalised assistance with engaging local employers for work experience.
  • Build rapport with participants from diverse range of backgrounds with the ability to engage and empower people to achieve their goals
  • Provide and maintain an environment that is free from any form of abuse, neglect and exploitation to ensure that the rights of the individuals are being upheld and maintained at all times.
  • Provide effective service delivery in line with the organisation's policies and procedures.
  • Maintain confidentiality in accordance with the Privacy Act.
  • Maintain appropriate records and data entry in accordance with the National Disability Insurance Scheme and Community Solutions' requirements.
  • Complete all administration requirements within a timely manner to ensure efficient operations of the service comply with Community Solutions Occupational Health and Safety policies and procedures.

What you will bring to the role

  • Must have a valid Driver's License and reliable vehicle
  • Ability to work effectively with individuals and small groups
  • Commitment to privacy and confidentiality
  • Good computer skills and experience using Word, Excel and Outlook

Additional requirements:

  • Right to Work in Australia i.e., Australian/NZ Citizen, Permanent Resident or Visa with permission to work excludes visas with Temporary Work Rights
  • A current
    Blue Card and
    NDIS Worker's Screening Check, or the willingness to gain these clearances if successful
  • A current
    First Aid and
    CPR certificate
  • A current and valid
    Driver's Licence:
  • Willingness to undertake a
    "Fit2Work" Police Check
