The opportunity
Our client is seeking an experienced and dedicated Care Manager to lead their aged care facilities in south-east Melbourne.

  • Flexible shifts from Monday to Sunday
  • Fulltime or parttime options available (minimum 4 days per week)

Key responsibilities include:

  • Undertake allocated admissions, including complex cases.
  • Utilise multiple approaches for clinical decisionmaking.
  • Coordinate the care of clients and their carers.
  • Establish therapeutic and professional relationships with clients and carers.
  • Engage in collaborative practice with peers and team members to achieve client outcomes.
  • Foster a supportive team environment and encourage professional development.
  • Adhere to legislation and common law that impact nursing practice.
  • Provide services in line with funding guidelines for project initiatives.
  • Current AHPRA registration as a Registered Nurse
  • Minimum 2 years' experience in acute nursing
  • Leadership experience as a Nurse In-Charge (desired)
  • Excellent communication, organisational, and time management skills
  • Proven ability to work independently and as part of a team
  • Current driver's license and willingness to travel
  • Current Federal Police Check
