**Your Role
**As ACH Coordinator - Assistance with Care and Housing you'll be looking after the most important people in our organisation - our clients.

Your tasks will include:

  • Assess, plan, coordinate and review care delivery which focus on needs and rights of care recipient and their carers
  • Enable clients to maintain as much independence as possible, and encourage them to use their abilities and initiative to develop existing and new skills with referral across various programs
  • Report and evaluate the effects of care through review and assessments
  • Provide direct client care when required and refer clients to appropriate agencies in cases where long term or specialised intervention is necessary
  • Undertake Case Management of clients as required
  • Participate in and contribute to the planning processes as prescribed by the Organisation

Your Attributes

  • Manage and ensure the effective and efficient utilisation of the financial, human and physical resources allocated to the Service
  • In consultation with Branch Manager, plan and implement program strategies in accordance with the Service Agreement
  • Utilise prescribed databases for the collection, collation, analysis, and reporting of statistical data
  • Deliver efficient and effective coordination of service provision using a variety of computer programs
  • Liaise with the Branch Manager and provide assistance where necessary to meet service outcomes
  • Contribute and participate in project work/research relevant to community practice or as directed by the Branch Manager

Role Requirements

  • Tertiary Qualifications (as appropriate) and/or relevant experience in a healthrelated field (e.g. Health and Human services)
  • Experience in case management and client care coordination
  • Demonstrated sound verbal, written and interpersonal communication skills to communicate and work effectively with a broad range of people
  • Current Queensland driver's licence
  • Current Police Screening Certificate
  • Current First Aid and CPR Certificates
  • Current NDIS Worker Screening Check (Yellow Card) and Blue Card (Working with Children Check)
