Clinical Intake - Team Leader
Part Time, Permanent Hours per week

  • Colac Area Health is a values based organisation providing a unique integrated health service comprising of over 500 employees, providing Acute Care, Aged Care, Community and Allied Health Services.
  • The Clinical Intake Team Leader provides and coordinates the clinical intake team via a single point of entry and coordinated referral and care pathways within eligibility, prioritisation and access guidelines that enhance person centred community based care and better health outcomes. This position will focus on reducing clinical risk, improving clinical efficiencies, and reducing avoidable hospital admissions.

What's required?

Essential:

  • Qualification in Health, Administrative, IT, Data Management or other relevant qualification or experience.
  • Effective written, verbal and interpersonal communication skills.
  • Highly developed engagement, interviewing, and assessment skills with both clients and staff.
  • Demonstrated team leadership skills and change management skills.
  • Demonstrated ability in complex project development and implementation of new programs.
  • Ability to relate to and negotiate with people from diverse social and cultural backgrounds.
  • High level of IT competency.
  • Demonstrated ability to work independently, as well as within a multidisciplinary team environment, in the delivery of health services to the community.
  • Demonstrated ability to effectively communicate with service providers relating to client eligibility for services and potential needs for additional referrals.

Desirable:

  • Minimum degree level qualification in Allied Health, Nursing or other with clinical experience.
  • Knowledge of relevant Department of Health prioritisation and access policies, including Service Coordination Practice Manual, and Community Health Demand Management Toolkit.
