Position Summary

  • Improve the patient experience by providing care in the comfort of their own home
  • Assist patients to achieve their care goals and best possible level of health and function
  • Promote and support safe and early discharge from hospital settings


The successful applicant will work in partnership with a strongly committed team to provide high quality and efficient care and support to patients in the home environment.


  • Possess strong interpersonal skills and work well in a team environment
  • Have experience working with older adults in the community and/or hospital environment
  • Have good time management skills and demonstrate a flexible and adaptable approach
  • Are committed to providing safe and high quality care as instructed by allied health professionals

To be successful in this role you must have:

  • Allied Health Assistant Certificate III or IV or other appropriate qualification
  • Previous experience working in hospital or community settings
  • Computer literacy
  • A current Victorian drivers licence
