Regional Assessment Service (RAS) - Home Support Assessor - what's involved in the role?

We are searching for a Casual RAS Home Support Assessor to evaluate our customers in the Bundaberg region. You will schedule your own timetable for a good work-life balance. In this position, you will receive specialized training in the assessment process through an online platform named My Aged Care Learning Environment (MACLE), fully covered by Suncare.

As a RAS Home Support Assessor, you will utilize the My Aged Care, NSAF (National Screening Assessment Form) to conduct a face-to-face, in-home evaluation to pinpoint areas of risk or needed support. Subsequently, you will help the individual find solutions to maintain their independence by linking them with suitable service providers and local support. Building quick rapport, effective communication with diverse individuals, and identifying necessary support are crucial aspects of this role to facilitate the person's ability to lead a full and independent life.

About you:

The ideal candidate must hold a tertiary qualification in Social/Human Services or a related field, with expertise in personalized service planning, referral, care coordination, and a solid understanding of community sector support services. We aim to ensure our customers have a positive interaction with our team. Hence, our personnel should exhibit an outstanding attitude, friendliness, exceptional organizational skills, digital literacy, willingness to learn, ability to work independently, and primarily, the capability to listen and comprehend the customer's desires and needs. Prior experience in conducting aged care customer assessments and developing support plans is highly advantageous.

For this role, a valid Driver's license is mandatory, and using your vehicle for work, with appropriate insurance, is required. Possession of a CPR Certificate and National Police Check is essential. If you do not currently possess these, we can support you in fulfilling these prerequisites during the recruitment/onboarding phase.
