This is a fantastic opportunity for an Occupational Therapist interested in a diverse role in Community Aged Care on a Full-Time permanent basis.

Your responsibilities will include visiting clients in their homes, conducting comprehensive Occupational Therapy (OT) assessments, and interventions to enhance health, wellness, and independence, enabling them to live safely in the community.

Day-to-day tasks will involve:

• Performing thorough OT assessments for clients, including those with diverse and complex needs
• Delivering tailored OT services and programs, such as home assessments and guidance on daily activities with recommendations on home modifications, equipment prescriptions, care planning, and education
• Assisting in creating various community-based Health and Wellness programs and groups
• Involving clients and caregivers in developing personalized care plans focused on reablement and person-centered care
• Offering guidance, education, and supervision to community workers and staff to enhance support for our clients
• Handling individual applications for home modifications and equipment following program guidelines
• Collaborating with the broader allied health team, including nurses, physiotherapists, fitness instructors, and allied health assistants
• Engaging in face-to-face clinical work with minimal travel time between clients, allowing time for administrative tasks and reporting
• Maintaining regular communication with external agencies and suppliers to coordinate required services and products for clients as per policies and procedures

To excel in this role, you need to have:

• Tertiary qualification in Occupational Therapy with postgraduate experience
• Current AHPRA registration
• Dedication and a proactive attitude towards working with clients, families, carers, and advocates
• Expertise in equipment prescription, pressure care, home modifications, and falls prevention for older adults
• Knowledge of local and international wellness and reablement care models
• Experience in a community setting and familiarity with the aged and disability service system in the community health sector (desired but not mandatory)
• Valid Driver's License with a registered and insured vehicle
