About the Role
Barnardos are looking for a full-time 12 month contract Case Manager for the Community Adolescent Program (CAP) based in Bruce, 2617

The Case Manager will be responsible for;

  • Leading the care team in supporting the child or young person's placement including developing, implementing and reviewing care planning.
  • Providing case management and support functions in line with the Care Plan for young people.
  • Co-ordinating care teams of diverse service providers and supports required to sustain young people's placements or transition to independence.
  • Identifying, negotiating and co-ordinating the delivery of services to meet the young peoples' needs.
  • Developing and maintaining relationships with key people involved in a young person's life.
  • Supporting the implementation of young people's therapeutic care plans.
  • Mediating and negotiating with young people and their birth families, ensuring a sense of identity is developed consistently over time.
  • Managing budgets associated with the needs of young people.
  • Providing culturally sensitive service that is responsive to the needs of Aboriginal children and young people and those from diverse cultural backgrounds.
  • Committing to self-care in ensuring personal work/life balance, management of professional boundaries and attending professional and peer supervision and reflective practice.


About You
As a Case Manager, you will ideally have;

  • A relevant tertiary qualification in Social Work, Psychology, Early Childhood Specialist and/or related behavioural sciences degree level with relevant experience.
  • Two years' previous experience in child welfare / youth work / community services / residential care
  • Demonstrated knowledge and skills in working with families and promoting family centered strength-based practices.
  • Demonstrated case management experience including a complex/competing caseload.
  • Demonstrated expertise in responding creatively to the needs of children and young people with highly complex issues.
  • Sound knowledge of child protection and issues in relation to children at risk particularly in regard to their safety, development, and connection to family, culture and community
  • Excellent interpersonal, facilitation, presentation, communication and consultation skills enabling successfully negotiation, conflict resolution, liaison and marketing.
  • Ability to liaise with families, foster carers and professionals and to advocate for children/young people and carers.
  • High level of written communication skills, ability to prepare reports and correspondence to a high standard for a range of audiences.
  • Ability to engage and support young people as they transition into independence, including a specific focus on living skills development.
  • Be available for after-hours emergencies and participate in the on-call and duty roster.

Salary: Base $71,136 to $82,054 per annum plus leave loading plus superannuation. Packaging available to receive $15,860 of your salary tax free. Motor vehicle packaging available on selected roles.
