About the Role
Our client is looking for a Primary Care Nurse Practitioner who will play a pivotal role in delivering high-quality patient-centered care within the medical centers. The PCNP will work collaboratively with the multidisciplinary team to assess, diagnose, and treat a diverse range of acute and chronic health conditions. When not consulting the PCNP will fulfill the duties of a practice nurse, contributing to the efficient operation of the medical centers.
Nurse Practitioner Duties:
- Conducting Comprehensive health assessments, including physical examinations, medical histories and diagnostic tests.
- Formulating and implementing evidence-based treatment plans, including prescribing medications, ordering laboratory tests, and referring patients to specialists as needed.
- Providing patient education and counselling on disease prevention, health promotion and management of chronic conditions.
- Collaborating closely with the General Practitioners, Registered Nurses and administrative staff to ensure seamless coordination of care and optimal patient outcomes
- Participating in multidisciplinary team meetings, case conferences, and quality improvement initiatives to enhance patient care delivery and outcomes.
- Demonstrating a commitment to professional development by staying current with best practices, guidelines, and advancements in primary care and nurse practitioner practice.
- Upholding our client's mission and values by fostering a supportive work environment promoting a culture of caring, commitment and communication, and delivering exceptional healthcare to every patient.
About you:
- Current registration as a Nurse Practitioner with the Nursing and Midwifery Board of Australia.
- Advanced Clinical knowledge and skills in primary care with a minimum of 1 year of experience as a Nurse Practitioner preferred.
- Excellent communication and interpersonal skills, with the ability to effectively collaborate with patients, families and healthcare team members
- Strong critical thinking, problem-solving, and decision-making abilities, with a commitment to evidence-based practice and continuous quality improvement.
- ABILITY TO WORK AUTONOMOUSLY AND AS A PRT OF A collaborate team in a fast-0paced clinical environment
- Current CPR certification and professional indemnity insurance coverage
- Commitment to upholding our client's mission, values and standards of excellence in patient care.
